Tidy up PluginVmInstaller declaration order and header comments

plugin_vm_installer.h:
- Update declaration order to explicitly show installation flow,
and match style guide.
- Make a couple of internal functions private
- Update comments to be clearer/simpler/etc.

plugin_vm_installer.cc changes:
- Re-order function definitions in .cc to match header file
